Seriously? A google doc? Didn’t the military take over this medbed thing and why would you need to get on a waiting list thru google docs?
Has anyone done this on here and who gets the email? This is curious to me and makes me wonder if that is for real. I guess we shall see.

My understanding is you could inquire about Med Beds when making your Redemption Center appointment or ask about them during your appointment. Or, you might get an email.
Is it permissible to submit the name of another person?
My sister-in-law has dementia. She cannot submit an application nor attend a redemption appointment and understand what’s happening. She can’t abide by an NDA.

I agree the form shown may, or may not “be real”. My inner gut tells me this form will serve no formal registration process, yet will give data pertaining to data gathering?
Ok…I would list your sister number name either way her diagnosis of dementia. She will then be linked to your email address.

It has been said that you may include your family members at that time, in your appointment offer. The real Med Beds have no fee, & no “one time charge” to set up an appointment.
